Chronic Disease Management.


A large part of General Practice is involved with the care of patients who have longstanding (chronic) stable conditions. At the practice we provide services for all of the chronic diseases that are usually monitored and treated within the primary care setting. Appointments are usually with one of our practice nurses, who are fully trained to provide these services, either in special clinics (diabetes, asthma, COPD, heart disease) or in routine nursing appointments. Each condition is overseen by a lead nurse and a lead GP, both of whom will have a special interest in that condition.
