Register with us.
We are currently welcoming new patients who live within the Practice area onto our list.
In order to register as a patient, please come in to reception and simply ask for a registration form.
You will need to provide details about your last GP practice, ideally by bringing in your NHS medical card if you have one. This will allow us to obtain your medical records as quickly as possible from your previous Practice.
In addition you will be required to provide proof of identity. Please click here to view a list of acceptable forms of identification.
Once registered, you will become a patient on the Practice’s list. You will also then be sent a medical card stating which GP you have been allocated to by the PCT, but because we work together as a group practice you can request to see whichever clinician you wish.
Whilst we will endeavour to accommodate your request to see a particular clinician, where this is not possible you will be offered an explanation why. Please also note that if you insist on seeing a particular clinician this may result in a longer wait for an appointment.
